Eligibility
Patients are required to be a resident of Hamilton County for a minimum of 90 days, not covered under TennCare, Medicare, Liability Insurance (i.e. injuries from auto accidents), Workers Compensation Insurance or any other medical insurance provider. Patients must be able to show that their income is less than 200% of the Federal Poverty Level. Guidelines may change from time to time based on resources and needs.
The annual household income threshold of 200% of Federal Poverty Level currently is:
- Single $20,800
- Household of 2 28,000
- Household of 3 35,200
- Household of 4 42,400
The clinic primarily operates on an appointment basis. If you qualify, prescreening will enhance your ability to receive service. For prescreening, call the clinic at 855-8220 and set up a time to visit and discuss your situation with a screener. Patients will need to qualify annually.
